Savannah Workers Comp Panel of Physicians Form

The panel of physicians is a list of doctors that are approved by the employer’s insurance company to treat an employee who is injured on the job. This list must have at least six (6) physicians, including a minority physician and an orthopedic surgeon.

Where Can I Find the Panel of Physicians?

If you are injured on the job, you must ask your employer or HR department for a copy of the panel of physicians. Your employer is required by law to post an official notice from the State Board of Workers’ Compensation in a prominent area in your workplace. This notice states that your employer must provide the panel of physicians along with all of the necessary paperwork to report your accident, as well as material on filing a workers’ compensation claim when you request it.

If your employer fails to provide you with a copy of the panel of physicians, or refuses to show you the panel, you have the right to seek treatment from any doctor of your choice.
